Children's secretary Ed Balls said: "We will set out what we can do to get excellent individual services - Sure Start centres and mid-wives, schools and GPs, youth centres and youth offending teams - working together with parents and services co-located in schools to spot problems early, tackle barriers to learning and then act effectively. That is our vision for schools in the 21st century."
The plan will also set out a vision for partnership between parents and schools, particularly secondary schools.
Schools should set up Parents Councils to ensure that parents' voices are heard in the school and parents should be contacted by a member of staff before their child starts at secondary school.
Every child will also have a personal tutor who knows them and acts as a main contact for parents.
