Gareth Price, from County Durham, hanged himself in his cell at Lancaster Farms Young Offender Institution in January 2005, a day before he was due to be sentenced for rape. He died in hospital the next day.
The inquest's jury highlighted failures by all the agencies involved in Price's care including youth offending teams and the Prison Service. The jury concluded that these collective failings ultimately contributed to his death.
In his conclusion, coroner Dr James Adeley said he was appalled by the number of agencies and individuals who missed opportunities to help Price.
