From April, the first of three locality based health and children's services teams will be created, involving education, social work and health professionals.
They will work under the same management structure but continue being employed by their current employers, including Trafford Council and Trafford Primary Care Trust.
This will mean that many employees will no longer report directly to a senior manager from their own profession.
Chris Pratt, Trafford Council's corporate director of children and young people's services said: "We have a structure in place to ensure that every member of the new team has someone senior from their own profession that they can turn to for advice or with help in their professional development."
The reorganisation has already involved the creation of multi-agency teams specialising in looked-after children and those with disabilities.
