In a report launched today at the National Children's and Adult Services Conference the institute outlines a multi-agency model which aims to help directors, managers and frontline practitioners look at every factor that contributed to a serious case review. The model, which the SCIE tested in two pilot sites in England, has been adapted from accident investigation methods used in aviation, engineering and health.
Julie Jones, chief executive of SCIE, said: "When a child has been at risk of harm, organisations must look at every factor that contributed, including training, culture, relationships and resources. Using this model of learning could help children's services steer away from the cases of individual blame we have seen documented in the last decade."
