
In documents being presented to its staffing committee on 4 August, the council confirms that "as a result of the inspection outcome, Linda Cointepas, the previous deputy director of children and young people’s services, left the council’s service by mutual agreement".
The unannounced inspection report into the council’s contact, referral and assessment process was particularly critical of management within children’s services.
It found that a "significant proportion of recent cases" involved children being put at risk of harm.
Many cases were prematurely closed "without the full range of risks being recognised and appropriately assessed".
Staff failed to "effectively engage children and young people". This led to an "incomplete understanding of the risks to them and other children and young people".
The Ofsted report, which was released in June, added that "management oversight of this work was poor and failed to remedy these significant weaknesses".
