
The boy, referred to as child BG, and a friend were accosted by two individuals and stabbed in Northamptonshire in August 2021.
The two assailants, including a teenager in care who had been moved from his out of area placement 24 hours before the incident, said they intended to carry out a robbery as they believed child BG and his friend would potentially have money or drugs on them.
This altercation turned violent and child BG died from a stab wound to his chest at the age of 16, while his friend survived the incident.
The review, carried out by Northamptonshire Safeguarding Children Partnership, finds that child BG had a long been at risk of criminal exploitation and gang violence.
