The watchdog published Learning Lessons: Taking Action today (1 December), which looks at the first 50 serious case reviews assessed by Ofsted between 1 April 2007 and 31 March 2008. Serious case reviews are carried out by local safeguarding children boards following the death or serious injury of a child where abuse or neglect is known or suspected.
